Charles J. Lesh, MD Apartments, a 55 and Over Single-Site Supportive Housing Program, provides long-term or permanent housing where residents can access the support services they require to live successfully in the community.

Located in Batavia, New York, the Charles J. Lesh, MD Apartments has a total of 62 units; 10 units are reserved for the 55 and Over Supportive Housing Program.

Services provided:

Tenants will be assigned to a housing specialist on-site to assist with identifying service gaps and subsequent linkages in the community, providing advocacy, networking with service providers, and support in maintaining independence through creation of a housing support plan.

Admission Criteria:

Age 55 or older.
Enrolled in Medicaid.
Have a documented Chronic Condition or physical disability.
Be in a housing crisis without a permanent residence.
Possess independent living skills to maintain an apartment.
Have a desire to participate in goal planning to maintain independence in your housing.
Must meet eligibility requirements of the Low-Income Housing Tax Credit Program. Income and occupancy requirements apply.

Program includes:

Rental assistance – Tenant pays 30% of their adjusted income or Department of Human Services (DHS) shelter rate, an agency rental stipend pays the remainder of the rent.
Security deposit is not required.
A fully furnished apartment, including necessities such as linens, dishes, silverware, pots, pans, vacuum and cleaning supplies.
Heat, air conditioning, hot water and electric are included in the rent.

The buildings are 100 percent smoke free. There are designated smoking areas provided outdoors.
